Health Action CANNOT answer queries about 18+ cards.

To get your 18+ ID CARD, call into any NZ Post shop, and ask for the form.

You will need the following to complete the form:

  1. Signature from a Justice of the Peace (there's a list of registerd JPs in the Yellow Pages of the phone book, under Justices of the Peace - listed by local areas)
  2. A passport-photo of yourself (fairly recent)
  3. One form of "Primary ID" - either Passport, OR Drivers License, OR your full Birth Certificate.  (If you do not have one of these, check the form for other options.)
  4. One letter or bill with your name and address on it, (Must be on letterhead from one of the following: an account or statement from your Bank, Building Society, or Credit Card, or Phone or Electricity account, OR a letter from your Employer or someone at your Education Institute if you are studying.)
  5. You will also need to find someone to fill in the "Identifier statement" on the form.  This person has to be someone who has known you for over 1 year, NOT a relative and NOT living with you, NOT your partner or spouse, AND they must be over 20yrs old and hold a current passport or Drivers License.
  6. A $20 fee to pay when you take the form back to NZ Post Shop to "lodge" it.

After you have lodged it, we are told it usually takes about 3 weeks, but over Christmas and New Year there will be delays because of the public holidays.
If you think your card is taking too long to arrive, or you have any other queries, call: 0800 500 503.

There's more info and you can download the forms at the Hospitality Association NZ (HANZ) website
